Kostas has a lot of good data, but I haven't been able to find an analysis of it, only some the papers that I know of. wanted to see if my method could be adapted to it. Then I discovered a paper by Zukos of the Max Planck Institute in Germany. They have developed what call a "model of the effect pharmacokinetic parameters on plasma concentration curve". Based their model, Zukos was able to show that the plasma concentration of this drug is not as large or close to the threshold as others have claimed.
When I read this, my heart stopped. I knew that would be seeing the difference only in calculations, not day-to-day use. Still. That much of an effect, regardless any explanation, is certainly worrisome. However. this study has more significance for me.
That's the paper, which I will quote at some length.
Abstract– The pharmacokinetic parameters of diovan could depend on the particular drug (diacetyl or acetylsalicylic acid respectively) and the dose administered. Therefore, a multivariate analysis of the data from clinical trials is warranted. By including the plasma level as a parametric and semiparametric predictor of the pharmacokinetic parameters, to account for the changes in plasma volume and concentration for the effect of drug on clearance, a predictive model could be estimated. The predicted concentration profiles represent potential drug effect, as a function of the dosage level. We found that in subjects who used the dose of diovan at a concentration 100 mg, the profile closely corresponded to pharmacokinetic parameters determined from clinical studies, i.e., as a dose-dependent increase of the mean (C max ) and a decrease of the mean (AUC0-t ) plasma concentration from preloading to the peak mean plasma concentration (C max /AUC100 ) at time 0. However, compared to the clinical valsartan genericon 80 mg data, we found a significantly increased mean (C max /AUC100 ) in plasma half-life. fact, the pharmacokinetic parameters had a value close to the concentration-related maximum of pharmacokinetics. In conclusion, the plasma concentration of diovan was related to pharmacokinetic parameters in subjects who use a high dosage and thus have a higher mean plasma concentration. generic alternative to valsartan half-life is a better predictor of the pharmacokinetics than plasma concentration for diovan, since it is not affected by the use of different diodes.
In summary, they use a prediction model to show that the plasma concentration of drug does not rise in the dose-related way that is usually reported (as a large rise between 1 to 3 hours after dosing). It also includes a new equation (2.3) to calculate the plasma concentration <$%OUTSIDE_LINK_0%$> based on other variables (i.e. the plasma concentration as a function of the concentration from placebo or a previous dose). And they show that the predicted plasma concentration (2.3) is substantially higher than their published data (2.6). For most of the concentration-related values, predicted values are very close to the published values.
Here are some comments.
These findings are important because it will allow us to test the reliability of various pharmacokinetic models. Many these models have been suggested previously. We use a mathematical framework to calculate the concentration–phase relationships. This provides a non-technical but interesting way to perform such calculations, and we hope that this model could also be useful to other researchers. Many of these different methods are based on assumptions. And we argue that they should be tested systematically to see what really happens in practice. fact, for example, the assumptions used to calculate various dosing rates are often based on other assumptions. But the calculation should always be repeated. Many of these models also assume that the plasma concentration goes off scale at a time interval of few hours, rather than a minutes. This depends on the time window used for calculation. That said, this valsartan generic dosage method can be used to calculate other dosing intervals.
One thing they do say is that this not a test of the model themselves. It is a test of some assumptions about the plasma concentration-related maximum which was not included in the model. But they have another problem. I don't know what the assumptions are about plasma concentration distribution. And since the actual data shows a lower concentration at time 1 than other measurements, it seems that there is a chance these assumptions are wrong. So they doing another analysis, based on this new data.
This is the most important result of this study - there is no concentration-related maximum. The plasma concentration of diovan goes down to zero. But this doesn't mean that the concentration of drug goes off the scale, since they're still predicting a peak plasma concentration (3)
